Two panes, a hollow spacer between them, a butyl seal around the edge, and desiccant inside the spacer to keep the space dry.

How the fog got there. Years of hot days and cool nights flex the butyl until it opens. Moisture gets in, and the desiccant soaks it up like a bathtub filling. You see nothing until the tub is full, and then the water comes out on the glass. In Chapel Hill's older homes the seals have simply had more years of that flexing.
